What would cause pain in lower back that runs over hip and down the leg to the ankle?

Her MRI is consistent with her symptoms. Pain at the L-4/L-5 or L-5/S-1 can both produce low back with radiating leg pain. Unfortunately, it sounds like the degree of her condition might be be enough that it goes beyond what physical therapy can help. She should return to her physician to discuss other options.

It sounds like sciatica which is pain that travels down the leg caused by a condition in the lower back. The pain is most likely caused by the disc slippage and arthritis. I have a herniated disc at L5-S1 with similar symptoms. The 2 things that help my symptoms the most are regular visits to a chiropractor and using electric stim(I have a personal tens unit). I have received electric stim at the chiro and PT but having my personal unit allows me a lot more freedom to treat myself. I would suggest she discuss this with her dr and physical therapist. The other option is to see a specialist. I have to warn you in my experience pain mgmt will recommend a cortisone shot, an ortho surgeon will suggest surgery but I’m not sure about a neurologist.
